The values listed in the drop down box "Treatment Facility" on the "Incident Investigation" form, are populated based on the form preparer's input. In other words, the values are an aggregation of all values used in the past. 

You can scroll the through the drop down to see a list of facilities that have been entered in the past. If you do not see the facility you're looking for, leave the drop down box blank, then enter the information for the treatment facility into their respective boxes. When the form is saved, a new treatment facility will be created with the values you enter. 

You can select the treatment facility from the drop down box in the future. Just scroll through the drop down, click the treatment facility, and the respective fields will automatically fill for that facility.
